Vault QualityOne provides an HSE Incident Management capability to allow your organization to report, track, investigate, and manage safety, environmental, vehicle, or property damage incidents. HSE Incident Management allows your organization to generate OSHA 301, OSHA 300, and/or OSHA 300A logs for you to be compliant with OSHA Regulations and reporting purposes. Additionally, your organizations can track labor information on a monthly basis and conduct risk assessments with visibility into your organization’s risk trends and statistics. This allows your organization to develop a safety-first culture and become more data-centric in your approach toward safety. This capability is only available in QualityOne Vaults.

HSE Incident Management captures required information through the following defined Event Category:

  • Incident: Any unplanned event resulting in injury or illness to people, danger to health, and/or damage to property or the environment.
  • Near Miss: An event that could have resulted in an injury to illness to people, danger to health, and/or damage to property or the environment.
  • Hazard: An object or situation that has the potential to harm a person, the environment, or cause damage to property.

HSE Incident Management Objects

QualityOne provides the ability to manage details about HSE Incident Management through the HSE Event object. HSE Incident Management uses the following object and object types:

  • HSE Event: Represents health, safety, environmental, and vehicle or property damage incidents.
    • Injury Illness: This object type stores injury illness incident information.
    • Environmental Event: This object type stores environmental event incident information.
    • Vehicle: This object type stores vehicle incident information.
    • Property Damage: This object type stores property damage incident information.
  • Labor Information: Represents the captured labor information data.
  • HSE-Risk Event: Represents the join between HSE Event and Risk Event.

HSE Incident Management Object Page Layout

HSE Incident Management provides default object page layouts for the HSE Event and its object types to create and manage incidents and for the Labor Information object to create and track employee and contractor labor hours and headcount. You can customize object page layouts or create custom object page layouts from Admin > Configuration > Objects and select the appropriate HSE Incident Management object.

HSE Incident Management Workflow

The HSE Event Lifecycle includes the standard HSE Workflow which allows users to submit incidents for verification, investigation, and final reviews before closing the incident record. You can customize standard workflows or create custom workflows from Admin > Configuration > Object Workflows > HSE Workflow.

OSHA 301, OSHA 300 & OSHA 300A Logs

You can generate OSHA 301 logs by creating a document from a document template and populating document fields with matching values from fields in the HSE Event object type records. To generate an OSHA 301 log for an incident record, navigate to the appropriate OSHA section on the relevant HSE Event object type record and create a new document from a template. Once the log is generated, the log is visible in the same OSHA section.

OSHA 300 and 300A logs can be generated by running a report. You can review and export the logs into pre-configured Excel templates.

Reports & Dashboards

HSE Incident Management is built on the Vault platform and therefore contains reporting and dashboard capabilities specific to the HSE Event object. Contact your Customer Success Manager to access a configuration-migration-package to deploy HSE Event-specific reports and HSE Event-specific dashboards.

Mobile Incident Management

HSE Incident Management includes a mobile accompaniment to help users capture incidents efficiently by providing mobile access to log incident records remotely. Once logged successfully, the records appear in Vault for management and investigation.

To manage object data records for the HSE Incident Management objects, you must have a security profile that grants the appropriate object-level-permissions.
